HRSA Audit Insights:
Updates & FY 2026 Data Request List Takeaways

The Health Resources and Services Administration (HRSA) continues to release initial audit reports with repayment findings, including diversion findings, as we discussed earlier this year when we were midway through the fiscal year (FY) 2025 audit cycle (check out our first audit webinar held on April 9, 2025 via our webinars page). With FY 2026 audits now underway, this webinar will build on insights we shared in April and provide the latest information we have about the HRSA audit landscape. We will discuss key observations about diversion findings and implications for 340B compliance. We will analyze trends in duplicate discount findings, identify sources of OPAIS records inaccuracies, and explore strategies for strengthening compliance and audit preparedness. In conjunction with our discussion on findings, we will explain the importance of HRSA’s appeals process and share takeaways from successful appeals. We will also review relevant changes between HRSA’s FY 2025 and FY 2026 Audit Data Request List (DRL). The DRL identifies the data and documentation that hospitals are required to submit to HRSA before the audit visit. Changes to She DRL can identify new areas of audits.
