Medicare Part B Drug Payment Cuts to 340B Hospitals: Potential Impact and Preparing Comments

The Centers for Medicare & Medicaid Services (CMS) has proposed substantial reductions to Medicare Part B reimbursement for 340B drugs as part of its proposed changes to the Outpatient Prospective Payment System (OPPS) for 2027. The proposed cuts, based on CMS's recent survey of hospital drug acquisition costs, would apply to 340B separately payable Part B drugs administered by disproportionate share (DSH) hospitals, rural referral centers, and sole community hospitals, unless the hospital has a rural sole community hospital designation from Medicare. Rural SCHs, children’s hospitals and PPS-exempt cancer hospitals would be exempt from the proposed 340B payment reduction. In addition, the proposal would reduce payments for non-drug Part B services on an accelerated basis to recoup the enhanced payments made to all OPPS hospitals to offset 340B cuts made between 2018-2022. The proposed rule also contemplates a new billing modifier for non-340B OPPS claims. Hospitals that are not paid under the OPPS, including critical access hospitals and Maryland waiver hospitals, would not be impacted by the proposals.
We strongly encourage affected hospitals to submit comments to CMS by the August 31 deadline. This webinar will discuss the proposed changes and their potential impact on hospitals, and review 340B Health's comment template and other resources to help hospitals develop their comments to CMS.
